SUMMARY A unfrecuent case of Hemobilia appears in a 16 years old, mate pacient with the antecedent of hepatic trauma by fall- As is arrival, ecography reveals an important intrahepatic hematoma of 1500 cc during its interment attends with anemia, the volume of the hematoma, initially diminished of 1343 cc, increases abrupthy unthil reaching 2522 cc, confirmed by abdominal TAC: reason why it is drained surgically with diagnose: of giant intrahepatic hematoma in right lobe, the post surgical procedure diagnose in the scale of Couinaud. 19 days after the hepatic drainage the patient present pain in right hipocondrio icteria, hematemesis and melena. A new ecography report intraparenquimatic liquid collection in right hepatic lobe, vesical hidrops with clots oran infected collection Reason why the patient taken again to the operating room with the diagnose of hemobilia acute colecistitis y piocolecisto the surgical procedure finding confirms hemobilia and hemocolecisto. The evolution of the patient was satisfactory sending him home at fifth day.
