We report a 24 years old patient; he went to the Emergency Department of San Jose Hospital in Santiago de Chile, who had open abdominal trauma, by gunshot, so he underwent surgery. In the surgical procedure we found transfixing hepatic injury in the right lobe grade IV, between the segment V and VIII, plus hemoperitoneum + / - 2000 ml. We introduced a Sengstaken-Blakemore tube through this injury and the balloon was inflated with 120 ml of serum saline, this solution stopped the bleeding. The patient had a negative evolution; he presented a big hematoma in the liver, so we used selective embolization, subsequently the patient get well after 30 days of hospitalization.
