The craniotomy fronto-temporo-sphenoidal, which is called the pterional; allows exposure of fronto-temporo-parietal operculum and Sylviana fissure, which is the approach most commonly used in today's neurosurgery. It is one of the initial frames advent of microneurosurgery. Over the last decades the pterional craniotomy was modified by several authors, and also led to broader craniotomy, among which include the supra-orbital craniotomy and orbital-zygomatic craniotomy. This article provides a detailed description of the technique that is currently used to achieve them, with their own modifications made by our surgical team.
