Congenital hypothyroidism is usually asymptomatic in the neonate and a lack of early treatment leads to a significant delay of development, because of its relative prevalence and severity of the disease, the routine neonatal screening has been instituted in many countries.   In the present work we describe the case of a female newborn with congenital hypothyroidism diagnosed by daily screening performed in all neonates at the Santa Cruz Hospital.   The optimal management of congenital hypothyroidism requires early diagnosis and timely treatment to prevent a patient's abnormal neurological development.
