    Resumen

    Objetive.- To demonstrate the advantages of laparoscopic surgery, vs. open surgery in the Surgery Department of “Clínica de la Caja Petrolera de Salud La Paz”, evaluating the admission diagnosis, operative time, length of hospitalization, initiation of oral postoperative pain and morbidity mortality. Materials and Methods.- We performed a cohort study comparing open and laparoscopic appendectomy between June - 2006 November - 2007. 50 were by open technique and 19 by laparoscopy. For the study, they took 19 patients with similar characteristics in both groups. Results.- The average age was 37 years, mainly males (64%). Of the 19 cases operated by AL, the diagnosis in 16 was acute appendicitis, 1 SFID and 2 acute abdomen, and the 19 operated by AA, 14 had acute appendicitis, 3 SFID and 2 acute abdomen. The average operating time was 48 min in AL and 66 min in AA, the average length of stay was 2 days in LA and 5 in AA. AL patients started orally at 13 hrs. vs. 46 hrs. AA. There was one case of residual intra-abdominal abscess in LA and 0% mortality in both series. Conclusion.- Laparoscopic appendectomy once again showed advantages over open surgery. In this study allowed testing low morbidity, no wound infection, shorter operating time, shorter hospital stay, early onset of oral and minimal postoperative pain
