Systemic lupus erythematosus is an autoimmune disease that affects any tissue, organ or system. The pleuropulmonary manifestations are frequent and among them the pleurytis (with or without pleural effusion). A rare condition is called "shrunken lung syndrome" (shrinking lungs) or lupus "small lung". Here is a patient of 53 years old female with recent diagnosis of systemic lupus erythematosus, hospitalized for two weeks box of evolution characterized by having generalized myalgia, chest pain of medium intensity, middle effort dyspnea. Radiography and computed tomography of the chest showed decreased lung volume and spirometry a severe restrictive pattern. Patient improved with high doses of corticosteroids associated with salmeterol and fluticasone.
