    López A.- Epidural fentanilo to Postoperative Pain. BACKGROUND AND OBJECTIVES: The relief of postoperative pain constitutes one of the most impor n goals o anesthesiologists, since this pain is what patients fear most. Several negative economic and medical consequences might happen if this painful condition persists. So, new approaches are being constantly studied. Some research using fentani! showed good results. The objective of this research was to evalúate the effectiveness of epidural fentanil as an adjuvant local anesthetics in treating postoperative pain. METHODS: A prospective clinical trial was conducted to compare postoperative pain in patients submítted to Abdomi: Hysterectomy. They were assigned on two groups of 20 patients each. All of them received epidural anesthesia with Bupiva-caine 0.5°o lOOmg. On group I. it was added + fentanil 40 mcg and on group II placebo was used. The numerical pain scale was used to evalúate the pain at 3rd, 6th, 12th and 24th hour post-blockage. We also evaluated the total amount of IV analge-sics used in 24 hours, the moment of first solicitation for this analgesics, the total amount of IV sedation used at surgery and side-effects. RESULTS: The results showed a higher pain score at all moments evaluated to groups II, likewise a larger need for IV analgesics (tramadol 25mg in group I and 97,5mg in group II) and a sooner request for this medication (group 111 h and group II 8h). Sedation was also harder in group II, with more midazolam used. However, when the incidence of side-effects was accessed we observed more pacients with postoperative nauseas and vomits in group I. There was no cases of Central side-effects (as hallucinations, unpleasant feelings and so). CONCLUSIONS: Epidural fentanil is an effective drug on treatment of postoperative pain in patients submitted to total abdominal hysterectomy in associations with local anesthetics and opioids, prolonging and improving the quality of this analgesia. Small-doses are safe in concern to central side-effects, proving that fentanil is an excellent adjuvant in treating postoperative pain.
