Abstract Allergic enterocolitis, is a form of clinical presentation within the various types of allergic reactions to cow's milk protein not mediated by Immunoglobulin E, is characterized by being a severe condition with vomiting, paleness, lethargy, profuse diarrhea, bloating abdominal, acidemia and shock; Its incidence in the neonatal period is unknown and sometimes they are often classified as necrotizing enterocolitis. A 19-day-old neonate was described, with a history of ingesting formula and the onset in the following hours of symptoms compatible with allergic enterocolitis, which showed reproducibility of the effects of the challenge and improvement on withdrawal of the formula formulated. Although it is an unusual entity in the neonatal period, it should be considered in the diagnostic possibilities because it is a medical emergency, especially if there are antecedents that reinforce this possibility, in order to provide timely treatment actions.
