Abstract Rosai Dorfman's disease is a rare, lymph node predilection, of unknown origin, which shows overproduction of histiocytes under microscopy. It is a benign pathology, however, it can mimic a cancer. We present the case of a female patient of 70 years with tumor in the left breast of 3 months of evolution, clinical and imaging evidenced data of malignancy. The biopsy determined Rosai Dorfman's Disease. In a multidisciplinary consensus, due to the aggressive-ness of the lesion and the large tumor size in relation to small breast, simple mastectomy was decided with immediate breast reconstruction using a prosthesis ex-pander. The clinical case is presented given the uncommon mammary location with association to aggressive characteristics.
