Odontogenic infections are one of the most common conditions treated at the Maxillofacial Surgery Unit, This ones, treated on time, do not cause problems, its spread will depend on several factors including the underlying disease, virulence of microorganisms and lack of appropriate dental medical care, they can also causesevere systemic complications that will threaten the patient's life, one of them is acute mediastinitis, which is very uncommon, characterized by a spread because of the vicinity of the bacterial infection into the mediastinal space, which early diagnosis is to maintain a high degree of suspicion, especially in those patients with an infectious process in the cervicofacial area; Computed tomography is really useful for diagnosis treatment planning and patient following. The purpose of this clinical case is to show one of the most serious and rare complications of an odontogenic infection, which affects a young person with no history of systemic pathology whose diagnosis, treatment strategy , appropriate surgical management and multidisciplinary approach, led him to preserve life and thus reduce the high morbimortality of this pathology.
