ABSTRACT We report the case of a 48 year oíd man with no personal or relevant medical history that progressed to clinical manifestations of Mediter-ranean Spotted Fever, with rash, fever, and black spot as classic triad, in addition to headache and muscu-loskeletal pain, caused by Rickettsia conorii confirmed by serology. Med-iterranean Spotted Fever is endemic in Spain. It is described in the sci-entific literature so me risk, alt-hough it is low, of fulminant infec-tion and múltiple organ failure (he-patic, renal, hematologic, cardiovascular and pulmonary) with high mortality. In our case, the patient improved after the administration of empirical antibiotic therapy (doxycycline).
