This article wants to show an analysis of the peculiarities of the health paradigm shift in current development, which began in the decade of the 70 and focused on the need for radical changes in the westem health designo In this sense, it is done an analyfical description ofthe context development of the decade and the main contributions, giving a/so a renewal process characterization, begining fram paradigm in depletion based on disease, Alma Ata and its rale in the transformation, until the emergence of the strategy of Primary Health Care (PHC). It is a/so characterized the antipode of renewal, understood as the many criticisms to the change on the part of subjects of resistance. It is concluded, as a result of the theoretical discussion, that the substitution (re-construction) of a widespread and acceptedthinkingis very hard in its develop, anditsdynamics, is defined in terms of actors who are, in their dialectical and communicating capacity, facilitators or antipodes helped by institutions that represent them. The collective thinking contemporary westem hegemonic medical for more than " centuries, was based on hospitalcentrism merchandised, alienating, mechanistic and patopeta/obsequious, that as any structure was produced and re-produced within people guided by conservation of their status and privileges. In this context, the Declaration of Principies of Alma Ata was the spearhead of the collective thinking reconstruction of Westem healthcare, which allowed first questioning a conceptual system inoperative, and the establishment of strategic and methodological guidelines based on the anticipation of undesirable evento
